ISLAMABAD, Jan 04 (Online): In a new studyTrusted Source conducted at Tulane University in Louisiana and published in Communications Biology, researchers describe a novel remedy for a persistent inflammatory condition that can make it difficult for children to eat.<\/p>\n
Known as eosinophilic esophagitis (EoE), this condition is activated by food allergies or airborne allergens, leading to the accumulation of eosinophils, a specific type of white blood cell, in the esophageal lining.<\/p>\n
Consequently, the esophagus becomes shorter and its wall thickens, resulting in difficulties with swallowing and food becoming lodged in the throat.<\/p>\n
Detecting symptoms in children can be more challenging and it carries higher risks because feeding difficulties can result in malnutrition and weight loss as well as impacting growth.<\/p>\n
Eosinophilic esophagitis linked to protein IL-18<\/p>\n
In their study, researchers determined that EoE can be attributed to interleukin-18Trusted Source (IL-18), a protein that plays a role in the innate immune response and can lead to inflammation when produced in excess.<\/p>\n
When the body is exposed to a food allergen, it triggers a pathway responsible for controlling the innate immune system, leading to the release of proinflammatory proteins such as IL-18. This, in turn, gives rise to eosinophils that cause harm to the esophagus.<\/p>\n
The new research revealed that effectively blocking the NLRP3 pathway, along with the release of IL-18, prevented the onset of EoE triggered by both food and airborne allergens.<\/p>\n
Anil Mishra, PhD, a lead author of the study and the director of the Eosinophilic Disorder Center at the Tulane University School of Medicine, told Medical News Today that \u201ceosinophilic esophagitis (EoE) is a now well recognized global disease, prominent in the pediatric population and the prevalence have been gradually increasing over the course of the last three decades.\u201d<\/p>\n
The symptoms of esophageal dysfunction are a result of the disease, which is defined by a chronic inflammation and fibrosis of the epithelium and subepithelial esophageal tissues characterize that is strongly linked to food allergies.<\/p>\n

\nThe disease is \u201ccharacterized by progressive esophageal dysfunction that is localized to the esophagus and is dominated by eosinophils and associated with inflammatory or fibrotic blockages of the esophagus, which are present in most patients,\u201d he explained.<\/p>\n
Existing medication has the potential to inhibit this pathway<\/p>\n
The study, conducted in mice, identified a potential pathway called NLRP3\/caspase1\/IL-18 that plays a key role in EoE development, offering new insights into potential treatments for this condition.<\/p>\n
The researchers emphasized the significance of their proposed studies in light of the limited understanding of EoE\u2019s mechanisms and treatment options.<\/p>\n
They pointed out that further studies have the potential to greatly influence our understanding of the role of the NLRP3\/caspase1\/IL-18 pathway in the initiation of EoE pathogenesis.<\/p>\n
The research pinpointed an existing medication, VX-765, as a potential inhibitor that could be effective as a treatment in people.<\/p>\n
Notably, this inhibitor would selectively reduce the harmful eosinophils produced and altered by IL-18, while leaving unaffected the white blood cells generated by IL-5, a crucial protein for preserving innate immunity.<\/p>\n
The next phase would involve conducting clinical trials in humans to assess the treatment\u2019s safety and efficacy.<\/p>\n
Lauren Mahesri, RDN, LD, a pediatric dietitian who was not involved in the research, told Medical News Today that \u201ceosinophilic esophagitis (EoE) is a drastically under-researched condition that often results in kids following extremely restrictive elimination diets.\u201d<\/p>\n
\u201cBecause there is no effective and broadly used cure, treatment is often left on the shoulders of the patient to \u2018figure out for themselves\u2019 what foods are triggering their symptoms,\u201d she said.<\/p>\n
This study was an incredible start at tackling the root cause of the disease. The treatment option found would tackle the physiological puzzle pieces that are out of place, rather than sticking a band-aid on an existing problem.<\/p>\n

\nPotential to improve the quality of life of children with EoE<\/p>\n
Mahesri highlighted that \u201cchildren who suffer with EoE often struggle with their diet.\u201d
\n\u201cThe constant elimination diets often lead to poor intake, vitamin and mineral deficiencies, weight loss, and sometimes even malnutrition,\u201d she noted.<\/p>\n
\u201cThe evidence found in this article would drastically improve the quality of life for these children because frequent elimination diets would not be necessary,\u201d Mahesri added.<\/p>\n
Atul Tandon, PhD, the founder, president and chief executive officer of NeoBiotechnologies who was not involved in the research, told Medical News Today that \u201cthis paper sheds light on the potential implications of targeting the NLRP3\/caspase1\/IL-18 signaling pathway in the treatment of eosinophilic esophagitis (EoE).\u201d<\/p>\n
\u201cBy understanding the mechanisms behind the disease pathogenesis, researchers can develop inhibitors that may protect against the damaging immune response seen in EoE,\u201d he explained.<\/p>\n
If successful, this research could have significant implications for patients and the public.
